"I authorize FINSERV Group, Ltd. dba LeaseAdvice and autoleaseDirect to start a credit investigation based on the information voluntarily provided by me which is true and
correct, and to obtain a copy of my credit report which may be obtained in connection with this application for an extension of credit for which this application is being made. Upon request, I will be told if a credit report(s) was/were
obtained, and given the name and address of the credit reporting agency(ies) providing the report(s). Upon request, I will be provided a copy of this application and forward my application and credit information, and the decision
regarding the application, to me.
LeaseAdvice intends to share the applicant's credit information with one or more of the listed Financial Institutions (lessors). LeaseAdvice agrees that whenever the funding source declines to extend credit to the applicant, the financing source will send an adverse action letter directly to the applicant
California Applicants: The applicant, if married, may apply for a separate account.
Maine and New York Applicants: A consumer report may be requested in connection with this application and the applicant may, upon request, be informed whether or not a
consumer report was requested, and if a report was requested, informed of the name and address of the consumer reporting agency that furnished the report.
Ohio Applicants: The Ohio Laws against discrimination require all creditors make credit equally available to all credit worthy customers, and that credit reporting agencies
maintain separate credit histories on each individual upon request. The Ohio Civil Rights Commission administers compliance with this law.
Rhode Island Applicants: A consumer report may be requested in connection with this application.
Wisconsin Applicants: NOTICE: No provision of any marital property agreement, unilateral statement under Wisconsin Statute § 766.59, or court decree under Wisconsin Statute
§ 766.70 will adversely affect the interest of the creditor unless applicant furnished a copy of such agreement, statement or decree to the creditor, or the creditor has actual knowledge of such provision before credit is granted.
